This is an OPEN CONTINUOUS ANNOUNCEMENT. This announcement will remain open until September 30, 2024 (or when positions filled). The initial cut-off date for referral of eligible applications will be February 1, 2024. Duties:

  • The RN Mental Health Case Manager demonstrates clinical expertise, leadership, and creative approaches to management of mental health nursing care for general outpatient mental health patients with DSM 5 diagnoses within the Dorn VA Medical Center and Community Outpatient Clinics (CBOCs).\
  • Applies clinical skills of case management which emphasizes a collaborative process that assesses, advocates, plans, implements, coordinates, monitors and evaluates health care options and services so that they meet the needs of the individual patient.
  • The RN Mental Health Case Manager will follow discharged Veterans from inpatient psychiatric unit (VAMC or Community), new patients to the Mental Health Outpatient clinic, and those with history medication non-adherence, episodes of crisis including suicidal and/or homicidal ideations/plan and systemps that have impaired the functioning of the Veteran's activities of daily living.
  • Responsible for the monitoring the care of the Veteran and assist in reengaging Veterans who have discontinued treatment and the need for their return has been established.
  • Conduct new patient nursing assessments, entering nursing interventions in the comprehensive treatment plan and monitoring efficacy of psychiatric medications.
  • The RN wis an active member of multidisciplinary team and is on a rotation schedule to over the Medication Injection clinic.
  • Assist the providers in transitioning cvare to the emergency room for possible admission to the inpatient psychiatric unit.
  • Additional duites include being a Clinical Lead RN in absence of the Nursing Manager (NM), representing the NM on clinical practices committees' providing in-service training on areas where he/she are the subject matter expert and assisting in orientation of new staff members.
  • Responsible and accountable maintaining a caseload of general mental health clients with DSM 5 diagnoses.
