Position Summary:

The Caregiver (Care partner) will provide personalized care and services to residents, uphold the philosophy of assisted living, and promote the well-being of the residents. Caregivers will provide assistance and support with residents’ activities of daily living in a professional and caring manner. Duties performed by the Caregiver are expected to be in accordance with the current Federal, State, and local regulations, as well as, the guidelines outlined in Oaks Senior Living policies and the employee handbook.

Primary Responsibilities:

Resident Services

1. Provide quality care and assistance to residents in accordance with the Oaks Senior Living philosophy of Person-Centered Lifestyle and in accordance with resident rights.

2. Assist residents with the activities of daily living according to individual resident preferences.

3. Protect the confidentiality of personal and medical information of residents and coworkers.

4. Adhere to each resident’s Negotiated Service Plan; notify the Shift Supervisor, MOD, Wellness Director, and/or Executive Director of any changes in the resident’s condition.

5. Answer and respond immediately to resident calls.

6. Assist with resident activities, encourage, and assist residents to participate in activities.

7. Encourage residents to attend meals in the dining room, escort residents to and from the dining room, assist with serving meals, assist residents with meals in ways that meet their individual needs and desires, observe and report changes in the residents’ appetite and physical abilities to eat and drink.

8. Assist with cleaning the dining room after the meal service as assigned.

9. Assist with cleaning and tidying residents’ room/apartment.

10. Maintain clean and sanitary conditions in resident, employee, and public bathrooms as needed.

11. Establish and maintain good relationship with residents and their families.

12. Dispose of all incontinent products according to policy.

13. Practice proper body mechanics when assisting with resident transfers and follow safety guidelines according to the Oaks Senior Living policies and procedures.

14. Respond to resident emergencies following proper policy and procedure.

15. Offer choice, maintain dignity, and promote individuality for all residents.

16. Protect and maintain resident rights.

17. Obtain 24 hours of continuing education in initial year of employment and 16 hours annually, thereafter. Maintain CPR & First Aid certification.

18. Practice infection control policies and established safety precautions.

19. Participate in special events, activities, and marketing programs.

20. Carry out other duties as assigned by the Shift Supervisor, Wellness Director, MOD or Executive Director.

Communications

1. Attend all regular staff meetings and required in service training sessions.

2. Must communicate effectively to convey information to residents, Shift Supervisor, and Management.

3. Review daily any and all communication tools used in providing resident care.

4. Communicate residents’ needs with Shift Supervisor and caregiver staff at change of shift.

5. Notify your supervisor or the Wellness Director if you observe a change in a resident’s condition.

6. Report all supply needs.

7. Report hazardous conditions and equipment to the Executive Director.

8. Respond to on-the-job injuries in accordance with the community’s policies and procedures.

9. Notify your supervisor of any concerns or questions presented by the residents’ family members.

10. Maintain a positive attitude.

11. Other caregiver duties as assigned

Reports to: Shift Supervisor, Wellness Director, Manager-on-Duty, and Executive Director

Qualifications:

1. Experience in the care giving to older adults preferred.

2. Ability to read, write, and speak English.

3. Desire to work with older adults. Friendly, caring disposition.

4. Must be 18 years of age. Must have a satisfactory criminal history check.

5. Must have physical exam by a licensed physician. Must have a negative drug screen.

6. Must understand the risk of exposure to infectious waste, disease, chemicals, solution, and disinfectants.

7. Must be able to react in an emergency situation.

Physical Job Requirements:

To fulfill job requirements, employee must be able to perform key physical task for extended periods of time. Employees must adhere to proper body mechanics and safety measures when supporting or transferring residents. In a typical eight- hour day, employee will:

1. Stand/walk up to eight hours a day

2. Sit up to two hours a day

3. Frequently support up to 70 pounds

4. Occasionally lift/carry up to 50 pounds

5. Frequently kneel, bend, and reach

6. Secure proper assistance for transferring of residents as needed
