POSITION SUMMARY:
The Physical Therapist screens, evaluates and treats patients/residents, documents services and communicates with families and interdisciplinary team members while facilitating optimization of patient/resident mobility and activities of daily living.
Ā DUTIES & RESPONSIBILITIES:

Provides professional Physical Therapy services.
- Completes documentation accurately and timely.
- Participates in Quality Assurance and Performance Improvement initiatives.
- Works efficiently and effectively while utilizing facility resources responsibly.
- Maintains adequate and up to date knowledge of Physical Therapy, Rehabilitation Services and setting specific clinical and regulatory practices.
- Complies with facility policies and procedures.
- Supervises Physical Therapist Assistants, therapy aides, and therapy students as needed.
Ā QUALIFICATIONS & REQUIREMENTS:

Must be a graduate of an accredited Physical Therapy program.
- Must have one year experience as an Physical Therapist (geriatric rehabilitation setting preferred)
- Must have current license to practice as a Physical Therapist in facility-specific state
- Must have current certification in Basic Life Support.
- Reliable transportation required.
