RN ADMISSIONS $5k SIGN ON

DEPARTMENT: Nursing on Rehab, short term resident unit.

Must have current South Carolina Nursing license unencumbered and short term rehab or long term care experience. 

HOURS ARE 10:00 AM TO 7:00 PM (CAN BE FLEXIBLE DEPENDING ON WORK LOAD) MONDAY-FRIDAY.

$5,000 Sign on Bonus Available

THIS POSITION REPORTS TO: Director of Nursing

DUTIES:

• Works closely with facility Admissions Coordinator to review preadmission records. Makes courtesy / marketing visits to hospitals and other healthcare facilities, home visits, and visits to physician offices, as necessary, and as assigned.


• Coordinates the admission / readmission process by assessing readiness of the residents assigned room and any necessary equipment prior to resident’s arrival to the neighborhood.


• Identifies current active diagnoses on admission / readmission of residents   to the facility for assignment of ICD-10 codes by medical records.  


• Cooperates with other members of medical care team and personnel of other departments in providing for resident’s total needs.


• Oversees, and may assist with, the admission/readmission process.


• Review of admission/readmission charts with 24 hrs. of admission whenever possible but no later than 72 hrs. after admission.


• Visits newly admitted residents frequently to assist with transition to the healthcare setting.


• Assists in teaching resident’s good health habits.


• Input into the resident’s care planning process.


• Attend morning stand up meetings, staff meetings and in-service educations as assigned.


• Successfully completes a 90-day introductory period.


• Responsible for personal adherence to all company rules and policies.


• Serves as backup to the Admission Coordinator in his/her absence.


• All other duties as assigned by Director of Nursing.
