Job Summary
The Health Navigator works collaboratively with other care team members to deliver exceptional, thoughtful care navigation to a population with potentially complex clinical and social needs. Under direction from the MD, NP and/or RN manager, the Health Navigator will be responsible for outreach to community based organizations, screening, coordinating and case managing under resourced individuals. Functions effectively as a team member, collaborating with other health care professionals to facilitate resolutions to barriers associated with social determinants of health.Essential Functions

Works with patients from diverse backgrounds to help them reduce socio-economic barriers to health. Acts as a patient advocate providing education on available resources in the health system or community. Collaborates with team member to address barriers when outside their scope of practice.
Completes pre-determined assessment tools to identify potential care gaps. Incorporates the use of data from EMR and other approved sources.
Contributes to data analysis efforts to improve practice/system response to social determinants of health and care coordination efforts.
Inputs data into customer relationship data base.
Tracks overall care of patient- scheduling appointments, communication, referrals, imaging, and navigation within or outside the health system.
Seeks innovative care opportunities for patient population that improve outcomes and reduce cost.
Coordinates care with health center team to provide service in the center, in the home or mobile.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
Education - High School Diploma or GED
Experience - Two (2) years primary care experience

Required Certifications, Registrations, Licenses
CMA or LPN must be obtained within one (1) year of hire.

Computer skills (word processing, spreadsheets, database, and data entry)
Mathematical skills
