Position Overview:
Licensed Practical Nurse (LPN) overall responsibility is to assist providers with patient care by providing direct and indirect patient care in a clinical setting within the scope of LPN nursing practice. LPN will function as the primary clinical contact person for patients, family, health care team members, community resources and employees as necessary.

Patient Centered Responsibilities:
- Participate in team huddle to promote clear communication among care team and treating provider.
- Obtain complete medical history from patients, family history, record vital signs, prepare patient, equipment and supplies, and assist providers with patient exams and/or procedures. Request medical records as indicated.
- Collect health care data directly or by phone to assist in planning patient care.
- Review and enter phQ-9 Questionnaire prior to patient's visit with providers.
- Provide total nursing care as determined by the patient's age related physical, psychosocial, and cultural needs in accordance with established standards, policies of SMF.
- Provide case management for patient care. Evidenced by being patient advocate, patient resource and patient contact person.
- Perform data collection, review basic health history and documents subjective and objective data in EMR appropriately.
- Facilitate patient medication management based upon standing orders and protocols. Administers medications as ordered.
- Perform lab/diagnostic testing as ordered by provider and reports all critical lab values immediately to provider.
- Schedule and follow up on referrals and other diagnostic as ordered by provider in timely and professional manner.

Clinical Quality Responsibilities:
- Assist with coordinating care with physicians, nurses and other staff ensuring quality patient outcomes are achieved within established time frames and with efficient utilization of resources.
- Maintain confidentiality within guidelines of HIPPA and adhere to infection control polies and protocols.
- Participate in team problem-solving methods, communicate information efficiently and effectively and maintain cooperative rapport with staff members.
- Use universal precautions when obtaining specimens and as appropriate.
- Participate in CQI program of SMF as requested.
- Remain current with the trends and professional development.
- Exhibit professional conduct at all times.
