Be the Patient Assessment Standard Coordinator you always wanted to be

聽聽聽聽聽聽聽聽 Assure accurate data extraction from clinical documentation.
聽聽聽聽聽聽聽聽 Coordinate timely submission of data for Medicare patients.
聽聽聽聽聽聽聽聽 Educate and support staff on proper and accurate documentation.
聽聽聽聽聽聽聽聽 Act as the primary resource for problem-solving regarding Quality Indicator coding and IRF-PAI completion.
聽聽聽聽聽聽聽聽 Work to improve process of QI coding and all other data collection specific to IRF-PAI.
聽聽聽聽聽聽聽聽 Ensure IRF-PAI data is entered and transmitted accurately.
聽聽聽聽聽聽聽聽 Ensure IRF-PAI data is transmitted to CMS/UDS within time frames specified for admission/discharge.
聽聽聽聽聽聽聽聽 Review, interpret and collect data on each patient in preparation for completion of the IRF-PAI.
聽聽聽聽聽聽聽聽 Collaborate with team on identification of potential comorbidities or accurate CMG/RIC categories.
聽聽聽聽聽聽聽聽 Ensure all discharged patient records contain the required elements.
聽聽聽聽聽聽聽聽 Complete chart audit to allow for timely UDS export and CMS transmission.

Qualifications

聽聽聽聽聽聽聽聽 License or Certification:

o聽聽聽 Licensed or certified clinician in healthcare (RN, LPN, PT, PTA, OT, COTA, SLP, Recreational Therapist, Respiratory Therapist, case manager/social worker)
o聽聽聽 QI Credentialed
o聽聽聽 Obtain UDS IRF PPS Certification after 2 years in the PASC role and before the 3rd-year anniversary.

聽聽聽聽聽聽聽聽 Minimum Qualifications:

o聽聽聽 Ability to perform assessments.
o聽聽聽 1 year of healthcare experience.
o聽聽聽 Attend all EHC IRF-PAI trainings.
o聽聽聽 Preferred: Experience in rehab or post-acute care, proficient teaching, and communication skills
聽聽聽聽聽聽聽聽 Effective oral and written communication skills
聽聽聽聽聽聽聽聽 Strong organizational and critical thinking abilities
聽聽聽聽聽聽聽聽 Detail-oriented and capable of meeting deadlines independently
聽聽聽聽聽聽聽聽 Commitment to maintaining high-quality standards in patient assessments.
