Provides direct patient care to include assessment, planning, implementation and evaluation of assigned patients.Β Collaborates with other professional disciplines to ensure effective patient care delivery and achievement of desired patient outcomes.Β Β
Responsibilities- Performs and documents comprehensive systematic patient assessments.Β Identifies deviations from the norm and intervenes accordingly.Β Β
- Exercises scientific based clinical judgment and decision-making skills related to patient assessment, planning, implementation and evaluation.Β
- Initiates, develops and maintains a written plan of careΒ
- Coordinates and supports the plan of care through communication with other members of the health care team.Β
- Initiates interventions or actions based on patient condition and plan of care.Β
- Performs related duties as required.
QualificationsRequired:
- Education: Associateβs degree in Nursing required.
- Licensure and Certifications: Β
- A valid Permanent Multi-State RN License from the state in which you reside is required. Should you hold a RN license from another Compact state, you are required to apply for and obtain a Multi-State RN License from the state in which you reside within 30 days.Β
- American Heart Association BLS required, must be obtained within 7 days of hire.Β
- ACLS and NIHSS required within orientation period.Β
